Key Responsibilities- Provide ongoing, continuous care to assigned panel(s) of veterans
- Participate in team performance improvements and sustainment activities to optimize team efficiency and care delivery
- Implement primary care operations management processes as appropriate
- Manage communications and facilitate safe transitions of veterans between the PACT's site of care and other healthcare settings
- Provide health education and health coaching on wellness, disease prevention, chronic disease management, and self-management skills to veterans, family members, and caregivers
- Engage veterans in using healthcare services, encouraging them to involve family and caregivers
- Use formal and informal communications that are respectful, effective, timely, and bidirectional with all team members, including veterans and family/caregivers, to convey significant, clinically relevant information for the care of the veteran
- Manage clinic workflow, ensuring veterans are placed in examination rooms in a timely manner, and provide direction to veterans as they move through the clinic environment
- Responsible for maintaining knowledge of and skill in applying an intensive body of rules, procedures, and operations in telehealth-related technologies and a variety of computer hardware/software used in technology, data entry, retrieval, record maintenance, document, and report generation
- Prepare the clinical environment, which may include telecommunications equipment to be used for clinical examination and treatment
- Serve within their scope of practice as a tele presenter for WC or Telemedicine in tasks such as establishing and maintaining video connection, making introductions between patients and providers, and following the instructions of the clinician to facilitate an effective clinical encounter
- Take measures to ensure patient care is delivered safely by following appropriate procedures for cleaning of clinical equipment and care of environment by following all safety guidelines, protocols, and procedures
- Complete standardized clinical questionnaires and templates within scope of training and applicable guidelines
- May be required to perform specimen collection (urine, sputum, blood, or stool), ensuring to utilize proper labeling, packaging, and transport instructions from the Primary Care Community Based Clinics or assigned areas to the main facility laboratory
- United States citizenship
- English language proficiency
- Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC)
- Active, full, current, and unrestricted licensure as a Graduate Licensed Practical or Vocational Nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico), of the United States or the District of Columbia
- Completion of at least one year of qualifying experience at the GS-5 level or equivalent
This position requires the ability to work in a fast-paced environment and to prioritize tasks effectively. The successful candidate will be able to work independently and as part of a team, and will be able to communicate effectively with patients, family members, and other healthcare professionals.
