About the Role

Sweetser is seeking a skilled Nurse - Case Management Specialist to join our team. As a key member of our Adult Case Management Services department, you will play a vital role in promoting the integration of patients' medical and psychiatric healthcare, improving overall wellness, and enhancing the quality of life for our clients.

Key Responsibilities
  • Medication Management: Reconcile medications, provide education, and facilitate communication between medical providers and Community Integration Workers.
  • Medical Diagnosis Education: Provide education to staff and facilitate communication between medical providers and Community Integration Workers.
  • Collaboration and Consultation: Identify medical issues, collaborate with MDs/Psychiatric Medication Management providers/Community Integration Workers, and monitor medical and general health status of patients.
  • Supportive Counseling: Provide supportive counseling, listening, and problem-solving assistance to patients and their families.
  • Discharge Planning: Participate in discharge planning processes and continue to consult regarding patients when hospitalized.
Requirements
  • Education: Graduate from an accredited school of nursing.
  • Certifications and Licenses: Current RN license in the State of Maine and a valid state driver's license.
  • Experience: Previous experience working with adults with serious and persistent mental illness is preferred. Persons with lived experience in the Mental Health and/or Substance Abuse system are encouraged to apply.
