Job Overview

Position: Community Medical Practice - Provider Relations Specialist

Key Responsibilities:

  • Establish and nurture business collaborations with leaders in primary and specialty care practices, along with medical personnel and administrative teams.
  • Enhance referral volume across all service lines offered by Andwell Health Partners.
  • Initiate the referral-to-admission process for each patient that is referred.
  • Maintain and update profiles of customers and competitors, log daily call activities, and document follow-up plans with referral sources.

Qualifications:

  • Familiarity with Andwell's Home Health, Home Care, Hospice, and Palliative Care programs, in addition to Mobile Wound Care, Outpatient Therapy, Community Care Team, and Behavioral Health services.
  • A minimum of one year of recent clinical nursing experience is required.
  • At least three years of experience in direct sales, case management, or an administrative nursing role, demonstrating a successful track record of achieving growth.
