We are seeking a dedicated and dynamic Assertive Community Treatment (ACT) Case Manager to support adults facing significant mental health challenges. Key Responsibilities:

  • Conduct initial assessments and ongoing evaluations for referred clients.
  • Lead Individual Service Plan (ISP) meetings and create tailored care plans with team input, including crisis management strategies.
  • Ensure effective implementation of ISPs, tracking and documenting client progress.
  • Maintain consistent communication with team members and external professionals involved in client care, fostering a collaborative approach.
  • Identify and evaluate health-related risks, ensuring clients and families are informed about available community resources and services.
  • Advocate for the needs of clients and their families.
  • Provide support through regular phone and in-person meetings, including home visits and community outreach, accommodating clients' schedules.
  • Collaborate with clients' primary care and other healthcare providers.
  • Engage in individual and group supervision, ongoing training, and case discussions.

Qualifications:

Applicants should possess MHRT-C certification or be eligible for it, or hold credentials as a Certified Rehabilitation Counselor, Employment Specialist, or Licensed Alcohol and Drug Counselor. A Bachelor's degree from an accredited institution is preferred.
