Key Responsibilities
  • Conduct thorough health assessments in patients' homes, reviewing medical history, medication adherence, and social environment
  • Collaborate with primary care physicians, case managers, and other healthcare professionals to develop personalized care plans
  • Provide education and support to patients and their families, promoting healthy behaviors and disease management
  • Document all assessments and care plans in accordance with Matrix Medical Network's guidelines and protocols
  • Participate in ongoing professional development to stay current with best practices and industry developments
Requirements
  • Master's degree in Nursing or Physician Assistant studies
  • Current RN and NP/PA licensure in the state of practice, with prescription authority or ability to obtain
  • Board certification in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant
  • Strong communication and interpersonal skills, with ability to work effectively in a team environment
  • Ability to travel to patients' homes, with valid driver's license and adequate automobile insurance
