- Provide education and support to patients and their families, promoting healthy behaviors and disease management
- Document all assessments and care plans in accordance with Matrix Medical Network's guidelines and protocols
- Participate in ongoing professional development to stay current with best practices and industry developments
- Master's degree in Nursing or Physician Assistant studies
- Current RN and NP/PA licensure in the state of practice, with prescription authority or ability to obtain
- Board certification in a Matrix-approved specialty
- Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant
- Strong communication and interpersonal skills, with ability to work effectively in a team environment
- Ability to travel to patients' homes, with valid driver's license and adequate automobile insurance
