Position Overview

We are seeking a dedicated and collaborative Assertive Community Treatment (ACT) Case Manager to support adults facing significant mental health challenges. This role is crucial in delivering comprehensive community-based case management as part of a dynamic interdisciplinary team.

Key Responsibilities:

  • Conduct intake assessments for new clients and provide ongoing evaluations as necessary.
  • Lead Individual Service Plan (ISP) meetings, collaborating with team members to create tailored care plans that include crisis management strategies.
  • Monitor the implementation of ISPs, documenting client progress and maintaining communication with the care team.
  • Engage with other healthcare professionals, including psychiatrists, nurses, and vocational rehabilitation providers, to ensure holistic care.
  • Identify and address both physical and behavioral health risks, guiding clients and their families to appropriate community resources.
  • Advocate for clients' needs and provide support through regular contact and home visits.
  • Work flexible hours to accommodate clients' schedules, ensuring accessibility to services.
  • Participate in supervision, training, and case consultations to enhance professional development.

Qualifications:

The ideal candidate will possess either an MHRT-C certification or be eligible for it, or hold credentials as a Certified Rehabilitation Counselor, Employment Specialist, or Licensed Alcohol and Drug Counselor. A bachelor's degree from an accredited institution is preferred.
