Bilingual Case Manager

Job Summary:
We are seeking a highly motivated and detail-oriented Bilingual Case Manager to join our team. The successful candidate will be responsible for providing case management services to clients, assessing their needs, and developing plans to achieve their goals.
Key Responsibilities:
- Conduct intake interviews with clients to assess their needs and develop case plans
- Provide case management services to clients, including counseling, advocacy, and referrals
- Develop and maintain relationships with community resources and service providers
- Monitor and evaluate client progress, making adjustments to case plans as needed
- Maintain accurate and up-to-date records of client interactions and case plans
Requirements:
- Bachelor's degree in Social Work, Psychology, or related field
- 2+ years of experience in case management or a related field
- Fluency in English and Spanish
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
