Clinical Care Coordinator

We are seeking a highly skilled Clinical Care Coordinator to join our team at UnitedHealth Group. As a key member of our Senior Community Care (SCC) program, you will play a critical role in coordinating the delivery and provision of clinical care to our highest-risk health plan members.
Key Responsibilities- Collaborate with an interdisciplinary care team to provide comprehensive care to patients in a facility setting.
- Communicate and coordinate with patients, families, and healthcare providers to ensure seamless care transitions.
- Assist with benefit determination, therapy coordination, and quality measure tracking.
- Participate in facility-based meetings and contribute to the development of care plans.
- Assist with documentation, coding, and medication reconciliation to ensure accurate and complete patient records.
- Associate degree or higher in Nursing (RN) with 1+ year of clinical experience in an acute care setting.
- Ability to travel between facilities and work in a fast-paced environment.
- Strong communication and interpersonal skills with the ability to work effectively with diverse stakeholders.
