Job Overview

The Kennedy Collective Inc. is seeking a dedicated and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in supporting individuals with their daily needs, promoting their well-being, and advocating for their rights.

Key Responsibilities:

  • Provide direct support to individuals, promoting their choice and autonomy.
  • Coordinate activities to foster individual growth, development, and well-being.
  • Ensure that all services, programs, and guidelines outlined in an individual's ISP are implemented as assigned.
  • Supervise and coordinate daily living skills, including personal care, recreation, meal preparation, and household chores.
  • Administer medications in accordance with DDS regulations.
  • Advocate for individuals and teach them to advocate for themselves.
  • Ensure the protection of individuals' rights and safety by following HIPAA regulations and reporting any abuse or neglect.

Requirements:

  • High school diploma or equivalent.
  • Valid driver's license.
  • Knowledge and skill with Microsoft Office and the internet.
