RN Patient Care Manager

We are seeking a skilled RN Patient Care Manager to join our team at LHC Group. As a key member of our home health care team, you will play a vital role in ensuring the highest quality patient care and outcomes.
Responsibilities:
- Coordinate patient referrals and ensure timely evaluation by clinicians and therapists.
- Determine patient home health benefits, medical necessity, and ongoing insurance approvals.
- Assess patient needs and develop individualized care plans that meet eligibility criteria and physician orders.
- Oversee and update patient plans of care as necessary.
- Communicate with clinicians and other healthcare professionals to ensure effective care coordination.
Qualifications:
- Current RN licensure in the state of Texas.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle.
