Healthcare Manager

As a Healthcare Manager, you will play a critical role in supporting patients with high-risk health conditions navigate the healthcare system. Your expertise will empower patients to take control of their health, and your collaboration with multidisciplinary care teams will ensure a seamless patient experience.
Key Responsibilities:
- Identify patients at high risk and develop personalized care plans
- Support longitudinal care of patients with chronic conditions through assessment, medication reconciliation, and patient education
- Coordinate transitions of care from inpatient to outpatient settings
- Establish and review contingency plans
- Provide accurate and timely documentation in electronic medical records
- Participate in daily huddles and Patient Care Coordination meetings
Requirements:
- Bachelor's degree in Nursing or Associate's degree in Nursing with 4+ years of experience
- Registered Nurse with an active and unrestricted license to practice in the state of employment
- Current BLS certification
- 2+ years of experience in a physician's office, clinical, or hospital setting
- Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF, and IHD
- Excellent verbal and written skills
- Solid interpersonal skills
- Ability to interact productively with individuals and multidisciplinary teams
- Proficient computer skills to work efficiently with electronic medical records
- Excellent organizational and prioritization skills
Preferred Qualifications:
- Experience related to patient education and/or motivational interviewing skills and self-management goal setting
- Fluent written and verbal skills in English and Spanish
