Job Summary:

The Care Manager II is a critical role within CHRISTUS Health, responsible for ensuring seamless patient progression through the continuum of care. This position requires collaboration with patients, families, physicians, and multidisciplinary teams to develop and implement effective care plans.

Key Responsibilities:

Coordinate patient care progression throughout the continuum of care, ensuring efficient and cost-effective transitions.
Serve as a patient advocate, providing support and expertise in treatment decisions and end-of-life issues.
Implement and monitor patient care plans, ensuring effectiveness and appropriateness of services.
Identify and escalate local and system barriers impeding diagnostic or treatment progress, as well as quality and risk issues.
Collaborate with medical staff, nursing staff, and ancillary staff to eliminate barriers to efficient delivery of care.

Requirements:

Graduate of an accredited school of nursing (BSN preferred) or Masters Degree in Social Work (MSW) required.
Two or more years of clinical experience, with one year in the acute care setting preferred.
RN or LMSW licensure in the state of employment required for new hires.
Certification in Case Management preferred.
