Job Summary:

We are seeking a skilled and compassionate Home Health Care Professional to join our team at Tri-County Health Care. As a Private Duty Nursing Caregiver, you will provide high-quality patient care in the comfort of their own homes.

Key Responsibilities:

Deliver direct patient care as assigned, using established procedures in the patient's home environment.

Travel to patients' homes and manage their care plans according to physicians' instructions.

Maintain accurate, detailed reports and records on medical information, vital signs, or changes in patient's conditions.

Monitor patients' medication usage and results.

Document patients' medical and psychological histories, physical assessment results, diagnoses, treatment plans, prescriptions, or outcomes.

Evaluate patients' behavior to formulate diagnoses or assess treatments.

Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans.

Provide personal assistance, medical attention, emotional support, or other personal care to patients.

Train other Registered Nurses, coordinate with other home health professionals caring for the patient, and communicate the patient's needs to the family providing support.

Benefits:

We offer a competitive compensation package, including a sign-on bonus up to $9,000, LVN to RN Program Tuition Reimbursement, weekend/night differentials, and a full benefit package (health, vision, and dental).

Requirements:

Associate degree in Nursing (Required)

BLS Certification (Required)

Valid LVN license in the state of Texas (Required)
