Key Duties:
  • Design and execute care management strategies that merge physical and mental health needs, ensuring seamless care and client empowerment.
  • Support clients in navigating health and disability services, obtaining housing, transportation, education, employment, and community resources.
  • Deliver preventive education and interventions on various topics, including smoking cessation, nutrition, physical activity, and stress management.
  • Provide crisis intervention services, working closely with management and therapists to guarantee client safety and formulate effective safety plans.

Candidate Qualifications:
  • Must meet ODMHSAS criteria for Certified Behavioral Health Case Management II (CM II) or achieve certification within 60 days of employment.
  • Preferred minimum of one year experience in a mental health or healthcare environment.
  • Proficient in MS Word and general computer skills; excellent verbal and written communication abilities; capable of collaborating effectively within a multidisciplinary team.
  • Valid driver's license in Oklahoma, current automobile liability insurance, and CPR/First Aid certifications are mandatory.
