Position Overview:

The Nursing Care Coordinator plays a crucial role in managing the health and wellness of members with complex medical needs. This position involves developing individualized care strategies, evaluating patients' health requirements, and working closely with healthcare teams and family members. Responsibilities include both remote and face-to-face care management, which may encompass home assessments, clinic visits, and hospital consultations.

Key Responsibilities:

  • Design tailored care strategies for patients
  • Evaluate the medical, psychological, and social requirements of members
  • Perform medication reviews and reconciliations
  • Analyze medical test outcomes and treatment efficacy
  • Facilitate transitions of care across various healthcare environments
  • Communicate care strategies to all relevant healthcare providers
  • Engage in Quality Improvement initiatives
  • Document care management activities meticulously
  • Collaborate with multidisciplinary teams
  • Educate and support members and their families
  • Implement evidence-based practices in care management
  • Participate in community health boards and initiatives

Qualifications:
A Bachelor's Degree in Nursing along with a minimum of one year of professional nursing experience is required. Valid RN Certification in the state of practice is essential.
