The Nurse Care Coordinator is a key member of the Mercy team, responsible for providing support, mentorship, and guidance to those who provide direct patient care. This role is designed to facilitate an effective process of care delivery, ensuring patients receive timely, safe, effective, and high-quality care.
Key Responsibilities:
- Assess care requirements of individual patients and make recommendations based on severity of illness, intensity of service, and skill level.
- Coordinate patient care with bedside nurses, ensuring seamless transitions and optimal outcomes.
- Hold direct and indirect care providers accountable for their professional responsibilities and adherence to regulatory guidelines.
- Manage assigned inpatient and outpatient populations, promoting quality patient care, safety, and financial components.
- Collaborate with interdisciplinary teams to optimize performance and adoption of best practices.
- Perform duties and responsibilities in a manner consistent with Mercy's mission, values, and service standards.
Qualifications:
- Education: Bachelor of Science in Nursing preferred.
- Licensure: Current licensure in the state of employment.
- Experience: 2-3 years nursing experience.
- Certifications: BLS and ACLS within 90 days of hire/transfer.
