Position Type & Work Schedule:

This is a full-time role, averaging 35-40 hours per week. Workdays and hours may vary based on program requirements, with services provided Monday through Friday from 8:30 am to 3:30 pm.

Role Summary:

Community Living Supports (CLS) are designed to enhance or sustain personal self-sufficiency, aiding individuals in achieving their goals for community inclusion, independence, and productivity. Programs support individual objectives as outlined in their Individual Plan of Service (IPOS). These supports are delivered in participants' homes or various community locations (such as libraries, camps, and volunteer sites).

Key Responsibilities:

This list is not exhaustive and does not imply that these are the only duties to be performed by this employee. Responsibilities include:

  • Demonstrating a commitment to providing exceptional service to families, participants, and stakeholders.
  • Implementing goals from Person-Centered Plans (PCP) and other relevant plans by offering assistance, reminders, observation, guidance, and training.
  • Providing personal care and supporting program staff as necessary.
  • Ensuring participants have daily access to community activities as per monthly schedules.
  • Fostering an environment of gentleness, dignity, respect, and safety for all participants receiving Community Living Support.
  • Ensuring all participants are informed of their rights.
  • Facilitating safe transportation between program sites and community activities.
  • Maintaining communication with families and caregivers through participant communication logs.
  • Transporting participants to and from the program as needed.
  • Attending all scheduled staff meetings and any additional meetings or training as requested by the Community Integration Supervisor.
  • Assisting participants in and out of vehicles, utilizing wheelchair lifts when necessary, and ensuring proper safety measures are in place.
  • Reporting vehicle maintenance needs to the Community Integration Supervisor.
  • Adapting staff assignments based on program needs to ensure adequate coverage.
  • Maintaining cleanliness and sanitation of assigned vehicles and workspaces.
  • Performing additional tasks as assigned by the Community Integration Supervisor, Program Services Manager, or Director of Programs.

Qualifications:

  • A high school diploma or equivalent (GED).
  • Ability to read and write in English, perform basic math, and comprehend written instructions.
  • Capacity to work collaboratively within a team with minimal supervision.
  • Ability to interact respectfully and courteously with a diverse range of individuals, including those with disabilities, seniors, children, and colleagues.
  • Completion of all required initial and ongoing training, including CPR and first aid.
  • Ability to pass a drug and alcohol test and DOT physical examination.
  • Maintain necessary certifications and physical condition as required by the Michigan DOT.
  • Possess a valid Michigan Chauffeur Driver License with a clean driving record (less than six points).

Travel Requirements:

Willingness to travel up to 5% for meetings, training, and conferences; use of a company vehicle may be provided, with occasional use of a personal vehicle required.

Work Environment:

This role involves spending extended periods in a vehicle, with temperature conditions varying based on weather. Employees will also work outdoors in various climatic conditions as needed.

Physical Demands:

This position primarily involves sedentary work, with regular requirements to stand, walk, sit, talk, hear, and use hands for operating equipment. Occasional lifting of up to 50 lbs may be necessary, with frequent lifting of up to 25 lbs. Close visual acuity is required for tasks such as data analysis, extensive reading, and inspections. The role also necessitates the ability to navigate steps on the bus and maneuver occupied wheelchairs using lifts.
