Job Overview

We are seeking a proactive, compassionate, and organized individual to take on the role of Patient Care Coordinator. This position primarily involves the remote communication, engagement, and monitoring of patients facing various health challenges. Your key responsibility will be to understand and track patient symptoms, thereby improving the coordination of care between patients and their healthcare providers. Daily interactions with patients will be conducted through structured questionnaires, assessments, and screenings. The relationships you build will be rewarding and will significantly enhance the quality of care provided.

Key Responsibilities:

  • Engage with patients on a daily basis
  • Administer structured questionnaires and health evaluations
  • Maintain knowledge and awareness of diverse patient conditions
  • Collaborate with fellow Patient Care Coordinators to ensure seamless patient care coordination

Essential Qualifications:

  • Medical Assistant (MA) certification

Preferred Qualifications:

  • Certification in Case Management (CCM, ACM) upon hiring
  • 1 year of clinical experience in a healthcare organization, medical group, or hospital setting
  • 1 year of experience in utilization management or case management

Competitive Compensation: $14.00 USD per hour
