Job Overview

Position Summary: The Care Coordinator plays a pivotal role in managing and enhancing the quality of home health services. This position involves extracting clinical data from OASIS and various medical documentation, ensuring accurate ICD-10 coding for patient records in line with established protocols. The Care Coordinator also recommends improvements to OASIS documentation and provides guidance to clinicians based on clinical insights.

Key Responsibilities:

  1. Guarantees the safe and effective delivery of home health care services.
  2. Develops and oversees staffing strategies that reflect the productivity of both skilled and unskilled personnel.
  3. Coordinates and evaluates the quality of care provided by clinical and support staff.
  4. Acts as a liaison between the agency and medical offices, hospitals, and other external organizations to address client concerns, accept medical orders, and manage referrals.
  5. Assigns initial assessments to Registered Nurses, Physical Therapists, or Speech Therapists as necessary.
  6. Establishes methods for interdisciplinary care coordination.
  7. Monitors staffing patterns, missed appointments, and all scheduling-related functions.
  8. Reviews overtime, staffing discrepancies, and other operational metrics.
  9. Ensures all care authorizations are properly routed and utilized.
  10. Participates in the recruitment and onboarding of new staff members.
  11. Maintains up-to-date knowledge of healthcare services in home settings, as well as Medicare, Medicaid, insurance, and pre-authorization processes.

Qualifications: A graduate of an accredited nursing program is required, with a BSN preferred. A minimum of two years of experience in home health OASIS review and five years in home health or hospice is preferred. Certifications in home health coding (HCS-D or BCHH-C in ICD-10) and OASIS (COS-C or HCS-O) are essential. Experience in quality assurance or case management is advantageous.
