Position: Community Support Specialist - Housing Assistance Program

Employment Type: Full Time

Compensation: $19.00 Hourly Non-Exempt

Work Schedule: Monday - Friday, 9AM – 5:00PM (37.50 hours per week)

ROLE SUMMARY:

The Community Support Specialist operates under the guidance of the Director to deliver service coordination and supportive counseling to participants within the housing assistance initiative. The primary objective of this program is to aid participants in accessing essential services, including referrals to various programs, assistance with entitlement applications, and connecting with resources that enable them to secure permanent housing and conclude their experience of homelessness. While the specialist works independently towards these objectives, consultation with the program director is encouraged as necessary.

KEY RESPONSIBILITIES:Service Coordination - Participant Engagement
  • Conduct individual assessments to identify participant needs and assist them in achieving their objectives, focusing on income, suitable housing, and additional supportive services.
  • Facilitate the process of finding and securing housing while fostering positive landlord relationships, empowering clients to enhance their independent living skills.
  • Provide referrals to relevant services, including drug and alcohol, medical, mental health, housing, vocational, educational, and family support services, while enhancing knowledge of CHS and other providers for client benefit.
  • Establish supportive relationships with participants to help them improve their self-awareness and understanding of key issues such as respect for diversity, individual uniqueness, and relationship building.
  • Advocate for participants to access appropriate entitlements and services from various agencies.
  • Encourage clients to engage with community resources and foster community connections.
Service Coordination - Administrative Duties
  • Maintain accurate progress notes in both the CHS database and physical records.
  • Handle paperwork related to fiscal responsibilities and the CHS database.
  • Generate statistical and written reports as requested by the supervisor.
Service Coordination - Programmatic Engagement
  • Participate in scheduled staff meetings and other relevant gatherings.
  • Communicate effectively with CHS staff, other service providers, and landlord/building management regarding participant matters.
  • Maintain open lines of communication and collaborate with CHS staff to achieve agency and program goals.
  • Support other homeless assistance staff, both management and program, as required.
  • Consult with the program director on strategies to enhance program effectiveness in alignment with agency and program missions.
  • Network with external agencies to secure appropriate services for all program participants.
QUALIFICATIONS:
  1. Bachelor's Degree with 1 year of experience in social services or 2 years of case management/service coordination experience, or a combination of education and experience.
  2. Strong interpersonal and problem-solving abilities.
  3. Empathy towards individuals and families seeking program services, particularly those facing behavioral, mental, and physical health challenges.
  4. Exceptional oral and written communication skills.
  5. Ability to prioritize tasks and work independently.
  6. Capacity to maintain professional boundaries.
  7. Proficient in scheduling and managing multiple responsibilities efficiently.
