Position Overview
The role of the Care Manager RN is integral to the collaborative care model, which involves a partnership among patients, nursing staff, social workers, physicians, and the community. This position emphasizes the importance of effective communication, both verbal and written, to ensure seamless care delivery through proficient resource management.
Key Responsibilities
- Advocate for patients to help them achieve optimal health outcomes.
- Assist patients in navigating healthcare services and utilizing resources effectively.
- Conduct thorough assessments and develop personalized care plans.
- Implement and coordinate care strategies, ensuring ongoing monitoring and evaluation.
Qualifications Required:
- Completion of an accredited nursing program.
- Active California Registered Nurse License upon hire.
- A minimum of 3 years of clinical experience in healthcare settings such as hospitals or medical offices.
Preferred Qualifications:
- Bachelor's Degree in Nursing or a related field.
- Master's Degree in a relevant discipline.
- Certification in Case Management (CCM, ACM) upon hire.
- Specialty certifications in areas such as Oncology, Rehabilitation, Pediatrics, Transplant, or Wound Care.
- Experience in utilization management or case management for at least 3 years.
