Job Summary

Amergis is seeking a skilled LPN/LVN Case Manager to join our team. As a Case Manager, you will play a critical role in coordinating continuum of care activities for assigned patients and ensuring optimum utilization of resources, service delivery, and compliance with medical regime.

Key Responsibilities
  • Case Management: Coordinate continuum of care activities for assigned patients, ensuring optimum utilization of resources, service delivery, and compliance with medical regime.
  • Documentation: Document patient case information within a database system, ensuring accuracy and timeliness.
  • Chart Review: Perform chart review/audits monthly or as needed, identifying areas for improvement and implementing changes as necessary.
  • Collaboration: Participate in monthly case conferences, providing information pertinent to patient's needs/goals and collaborating with the Program Director to develop and review individualized coordination of care plans.
  • Communication: Ensure that patients' medical needs are addressed, consulting with patients' physicians as needed, coordinating plans of treatment, and advocating for patients when necessary.
  • Patient Education: Promote understanding of medical factors affecting targeted populations, identifying and assisting patients in accessing entitlements, resources, information, and referrals for psychosocial needs.
  • Quality Assurance: Participate in Quality Assurance and Utilization Review activities, as directed, to ensure compliance with regulatory requirements and optimize service delivery.
  • Empowerment: Empower patients in decision-making for care planning, maintaining accurate and timely patient information, and assisting in data collection for reporting/funding sources.
  • Interagency Collaboration: Foster intra-facility and inter-facility working relationships to help accomplish goals, acting as a liaison between primary care providers, specialists, and/or patients.
  • Advocacy: Advocate on behalf of patients regarding accessibility of services, following State/Country mandated guidelines for the nurse case management programs.
  • Outreach: Participate in outreach activities to the entire target population, as directed, to promote services and identify areas for improvement.
  • Program Development: Recommend program/service changes to meet gaps in service in the community, performing other duties as assigned/necessary.
Requirements
  • Licensure: Current LPN/LVN licensure in state practicing.
  • Experience: At least one year of Case Management experience preferred.
  • Professional Standards: Comply with all relevant professional standards of practice.
  • Training: Participation and completion of Amergis' Competency program when applicable.
  • Certifications: Current CPR if applicable, TB questionnaire, PPD or chest x-ray if applicable, and Current Health certificate (per contract or state regulation).
  • Regulatory Compliance: Must meet all federal, state, and local requirements.
  • Training and Development: Successful completion of new hire training as applicable to job site.
  • Confidentiality: Understand patient confidentiality and HIPAA requirements.
  • Communication Skills: Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required.
  • Computer Skills: Computer proficiency required.
  • Age Requirement: Must be at least 18 years of age.
