Job Summary

The Illumination Foundation is seeking a highly skilled Medical Coordinator to join our team. As a Medical Coordinator, you will play a critical role in delivering high-quality medical care to patients experiencing homelessness in our recuperative care setting.

Key Responsibilities
  • Client Care:
    • Conduct initial intake assessments to ensure an appropriate care plan is established based on discharge information from the hospital or primary care notes.
    • Determine the patient's insurance status, make follow-up appointments with patient's providers, and review medication lists.
    • Establish and maintain a care plan, setting weekly goals for patients to complete to assist with medical or social reintegration, verifying goal completion, and assessing medical needs.
    • Conduct morning, afternoon, and evening checks for all assigned clients to observe wellbeing, ensuring there is no change in medical conditions since hospital discharge.
    • Link clients to medical services when necessary (insurance, PCP, specialists, referrals).
    • Coordinate home health visits (if needed) and follow-up on medical appointments.
    • Promote health literacy by providing patient education with regards to clients' illnesses, medications, preventative measures, treatment management, and overall general health.
    • Arrange transportation for clients to and from medical appointments, as needed.
    • Report emergency situations to Supervisor/Manager and document incident.
  • Administrative/Record Keeping:
    • Enter patient information and case notes into electronic health records and other documentation systems.
    • Complete and update medication, vitals, progress notes, and care plans.
    • Perform administrative tasks such as answering phones and filing paperwork.
    • Prepare patient reports for billing.
    • Maintain proper inventory of medical supplies and medications.
  • Compliance/Safety:
    • Maintain confidentiality in accordance with HIPAA guidelines, Illumination Foundation policies, and DHS standards.
    • Provide patients with proper compliance forms upon admission.
    • Utilize infection control measures according to OSHA standards.
    • Conduct medication reconciliation and ensure proper disposal and pickup of bio-medical/pharmaceutical waste.
    • Maintain a safe work environment by ensuring the cleanliness of the med room, wound care room, and medication storage room.
    Requirements
    • Current and non-restricted LVN license in California.
    • 2 years of relevant experience.
    • High School Diploma or equivalent.
    • Basic computer skills, including the ability to send and receive emails and summarize data in spreadsheets.
    • Valid CA Driver's License.
    Preferred Qualifications
    • Home Health nursing experience and experience working with populations dealing with addiction and behavioral health conditions.
    • Bilingual in English and Spanish.
    • Proficiency in Google Suite (Mail, Docs, Sheets, Calendar) and KIPU or other electronic health record systems.
