Position Overview
The role of the Care Manager is integral to our collaborative healthcare model, which involves a partnership among patients, nurses, social workers, physicians, and the community. This process emphasizes effective communication, both written and verbal, to ensure seamless care delivery through optimal resource management.
Key Responsibilities:
- Advocate for patients to achieve their health goals and facilitate access to necessary services.
- Conduct comprehensive assessments to identify individual healthcare needs.
- Develop and implement personalized care plans, ensuring coordination and monitoring of patient progress.
- Evaluate the effectiveness of care strategies and make adjustments as necessary.
Qualifications:
Required:
- Graduation from an accredited nursing program.
- Active California Registered Nurse License.
- A minimum of 3 years of clinical experience in a healthcare setting.
Preferred:
- Bachelor's Degree in Nursing or a related field.
- Master's Degree in a relevant discipline.
- Certification in Case Management (CCM, ACM) upon hire.
- Specialty Certification in areas such as Oncology, Rehabilitation, Pediatrics, Transplant, or Wound Care.
- Experience in utilization management or case management for at least 3 years.
