Overview
The RN Care Coordinator plays a crucial role in managing the continuity of care and discharge planning for patients in need of these essential services. This position is dedicated to addressing the individual health requirements of patients while ensuring high-quality care and cost-effective outcomes, all in compliance with hospital policies, standards of practice, and applicable Federal and State regulations. The focus will be on effective care coordination, clear communication, and collaboration with utilization management, nursing staff, physicians, ancillary departments, insurers, and post-acute service providers to achieve optimal patient outcomes at the appropriate level of care. The RN Care Coordinator serves as an advocate for patients and their families by recognizing and addressing their choices, spiritual needs, cultural, language, and socioeconomic barriers to care transitions.
Responsibilities
*$7,500 Sign-on Bonus
25% shift differential for floating to all Omaha/Council Bluffs Hospitals
  1. Conducts and documents a comprehensive discharge planning assessment for patients identified through the designated screening process or upon request. Reassesses patients as necessary and updates the plan accordingly.
  2. Facilitates the creation of a multidisciplinary discharge plan, engaging relevant health team members, the patient, and/or their representative, as well as post-acute care providers, in alignment with the patient's clinical or psychosocial needs, preferences, and available resources.
  3. Oversees and evaluates the execution of the discharge plan.
  4. Collaborates with the multidisciplinary team to ensure effective care progression and appropriate utilization of inpatient resources, utilizing established evidence-based guidelines and criteria.
  5. Works closely with the healthcare team and post-acute service providers to guarantee timely and seamless transitions to the most suitable type and setting of post-acute services based on the patient's clinical needs.
  6. Identifies potential readmission risks and implements strategies to mitigate these risks for a minimum of 30 days.
  7. Responsible for delivering necessary patient notifications and related documentation.
  8. Ensures patient education and advocacy are prioritized.
  9. Participates in performance improvement initiatives and programs as required.
  10. Exhibits behavior that aligns with the Mission and Core Values of the Organization.
  11. Completes required educational activities within established timeframes.
  12. Adheres to all hospital policies, standards of practice, and Federal or State regulations relevant to their practice.
  13. Performs additional duties as assigned, including utilization review as necessary.
Qualifications
*Required
+ Graduate of an accredited nursing program.
+ Minimum of two (2) years of clinical experience in an acute hospital setting or a Master's degree in Case Management or Nursing in lieu of one year of experience.
+ RN license in the applicable state(s) is required.
+ BLS certification required within three months of hiring if located within a hospital.

Preferred
+ Bachelor's Degree in Nursing (BSN) or a related healthcare field.
+ At least five (5) years of nursing experience.
+ Certification as a Case Manager (CCM), Accredited Case Manager (ACM-RN), or UM Certification is preferred.
+ Ability to apply clinical guidelines to ensure effective care progression.
+ Familiarity with managed care and payer environments is preferred.
+ Strong critical thinking and problem-solving skills.
+ Effective collaboration with multiple stakeholders.
+ Professional communication skills.
+ Understanding of how utilization management and case management programs integrate.
+ Ability to work collaboratively as a team player and assist other team members as needed.
+ Capacity to thrive in a fast-paced, self-directed environment.
+ Knowledge of CMS standards and requirements.
+ Proficient in prioritizing tasks and delegating as appropriate.
+ Highly organized with excellent time management skills.

Pay Range: **$47.43 /hour
