Our services include assistance with daily living activities, community engagement, job coaching, life skills training, and skilled nursing care as required.

Understanding the Role of a Licensed Practical Nurse Case Coordinator

In this position, you will manage the nursing processes for individuals served in your area. Working in tandem with the RN Case Coordinator, you will oversee a caseload of approximately 200 individuals, traveling as needed to train staff, perform routine healthcare tasks, and document any medical issues.

A Typical Day Includes:
  • Flexible scheduling (Monday to Friday, with on-call duties as necessary)
  • Conducting nursing reviews to ensure individual needs are met
  • Reviewing medical documentation completed by Direct Support Professionals and Managers
  • Administering routine healthcare tasks and specialized care procedures
  • Documenting reviews and medical issues, reporting any changes to the RN Case Coordinator
  • Acting as a medical liaison and consultant
  • Traveling up to 50% of the time within the designated region to check in on individuals served
  • Reviewing appointment records, lab results, and diagnostic tests
Requirements for Consideration:
  • Strong work ethic
  • Minimum of 2 years of experience as an LPN
  • Experience in case management is preferred, but not mandatory
  • Valid LPN license in the state of Nebraska
  • Experience working with individuals with intellectual and developmental disabilities
  • Familiarity with Therap is a plus
  • Willingness to undergo a background check, physical examination, and drug screening
