Care Manager RN

HDE HOME CARE LLC is a family-owned business providing personalized home care services in the Pacific Northwest. We are seeking an experienced Registered Nurse to join our team as a Care Manager RN.
This is a full-time or part-time opportunity for a skilled nurse to train staff for in-home care and assess clients. The successful candidate will be responsible for maintaining client care documentation, including medication administration records, and supervising visits.
We offer a competitive salary of $50 per hour and a range of benefits, including 401(k) matching, dental insurance, and paid time off.
Key Responsibilities- Maintain accurate and up-to-date client care documentation, including assessments, service plans, and medication profiles.
- Supervise visits and provide guidance to HDE Management and Direct Support Professionals.
- Assist with new employee orientation and training, identifying staff development needs and planning in-service programs.
- Monitor the quality of care provided through random record review and consultation with clients and caregivers.
- Current Oregon State Board of Nursing RN license.
- Able to read, write, and understand English.
- Physically capable of performing job-related duties.
- Demonstrate mature, responsible character and possess communication skills to interact effectively with clients and agency staff.
