Overview

We are seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Geriatric Care Manager in Beaverton, OR. As a core member of our interdisciplinary team, you will provide high-quality care management, care coordination, and skilled care services to at-risk frail elder patients with complex medical and psychosocial issues.

Salary and Benefits

The estimated annual salary for this position is $64,800 - $102,264, depending on experience. In addition to a competitive salary, we offer a comprehensive benefits package, including medical, dental, vision, life insurance, disability insurance, time off benefits, and more.

Job Description

Care Management: You will work closely with patients, families, and healthcare providers to develop and implement individualized care plans that promote patient-centered care, improve health outcomes, and enhance quality of life.

Care Coordination: You will coordinate with internal and external stakeholders to ensure seamless transitions of care, manage referrals, and facilitate access to community resources and services.

Skilled Care Services: You will provide direct care services, such as wound care, medication management, and vital sign monitoring, to patients in their homes or in clinics.

Patient-Centered Approach: You will collaborate with the interdisciplinary team to deliver patient-centered care that addresses the physical, emotional, and social needs of each patient.

Required Skills and Qualifications
  • Oregon Registered Nurse License
  • National Provider BLS - American Heart Association within 30 days of hire
  • Relevant certifications applicable to the position, as specified by the Nurse Manager
