Position Summary

As an ACT Team Lead, you will play a pivotal role in guiding and supporting a team of dedicated professionals who provide comprehensive services to individuals within our program. Your leadership will be instrumental in empowering these individuals to live fulfilling lives aligned with their aspirations.

Essential Duties and Functions
  • Staff Supervision:
    • Provide coaching, mentorship, and performance support to direct care staff within your program.
    • Effectively organize, schedule, and distribute work assignments among your team members.
    • Ensure personnel are well-informed about relevant policies and procedures impacting the program or their roles.
    • Administer established policies and procedures as outlined by Resources for Human Development.
  • Direct Care:
    • Maintain a direct service caseload, actively engaging with individuals supported by the program.
    • Coordinate and oversee medical appointments, including attending appointments when necessary. Communicate effectively with nurses and doctors regarding individual's medical needs.
    • Foster positive communication and relationships with family members, guardians, staff, and Regional Office personnel regarding policies, issues, or concerns.
  • Administration:
    • Actively participate in the person-centered planning process, including attending meetings, providing input, coordinating training plans, ensuring documentation is complete, reviewing progress notes and daily notes, monitoring plan implementation, completing monthly reviews, and communicating with service coordinators.
    • Prepare and submit required reports and documentation related to individuals supported by your program.
    • Ensure compliance with all regulatory bodies.
    • Implement and complete continuous quality improvement activities.
    • Perform other duties as assigned.
Qualifications
  • Bachelor's Degree in a Human Services field, including but not limited to social work, psychology, or counseling.
  • Equivalent combination of education and experience may be considered.
