Job Description

The Medical Coordination Assistant will play a critical role in ensuring the optimal facilitation of medical care for homeless clients discharged from hospitals or other agencies contracted with Illumination Foundation Recuperative Care Centers.

Responsibilities include:

  • Completing initial medical assessments of clients upon arrival, including head-to-toe assessments, vital checks, patient education, determining insurance status, and making medical follow-up appointments with clients' doctors.
  • Inputting and updating client information and medical notes in the KIPU EMR system.
  • Conducting rounds to remind clients of medication, assisting with checking blood sugar levels, and providing minor wound care teaching as needed.
  • Ensuring the success of a client's hospital discharge by confirming hospital instructions and checking medication.
  • Promoting health literacy by providing patient education on medical treatment management, diet, and personal health care procedures.
  • Collaborating with Medical Coordinators and Health Care Providers to ensure best patient outcomes.
  • Serving as an advocate for clients to ensure best medical care is provided by linking clients to medical resources and MediCare or Medi-Cal as needed.

Preferred qualifications include:

  • 2 years of relevant experience.
  • Bilingual in English and Spanish.
  • Proficiency in Microsoft Office Suite (Outlook, Word, Excel, Teams).
  • Valid CA Driver's License.
