Veteran Care Coordinator

As a Veteran Care Coordinator, you will play a crucial role in ensuring the well-being of veterans through comprehensive care management.
Key Responsibilities:- Act as the primary point of contact for the coordination of care for veterans within your caseload.
- Deliver recovery-oriented services, including assessments, case management, crisis intervention, group training, and skills development.
- Create personalized treatment plans, monitor progress, and provide ongoing support through individual counseling and group sessions.
- Oversee daily activities of participants, coordinating necessary medical, legal, dental, and psychological services.
- Assist residents in planning their developmental phases, focusing on job training, education, financial management, public benefits, housing, and mental health resources.
- For clinical participants, evaluate substance abuse issues and assist in determining treatment paths.
- Collaborate effectively with treatment team members to ensure a cohesive approach to care aligned with mental health recovery principles.
- Conduct pre-placement screenings to maximize housing retention and appropriate supportive services.
- Complete all required documentation to comply with program and funding guidelines, including intake documents and progress notes.
- Ensure the accuracy and quality of client records, tracking outcomes related to housing, case management, and client progress.
- Maintain client files using the appropriate data management systems.
- Communicate effectively with management and relevant stakeholders regarding service delivery challenges.
- Engage in professional development through conferences, workshops, and training sessions.
- Exceptional organizational and time management skills, with the ability to multitask and demonstrate initiative.
- Proficient in motivational interviewing and capable of working within a harm reduction framework.
- Knowledgeable about community resources related to substance abuse and experienced in multidisciplinary team collaboration.
- Able to interact with a diverse range of clients and staff from various cultural and economic backgrounds.
- Skilled in using basic office software and social services data collection programs.
- A Bachelor’s degree in a relevant field is preferred, though equivalent work experience may be considered.
- Possession of a Drug and Alcohol Certification or the ability to obtain one within six months of hire for clinical roles.
- A minimum of three years of experience in social model recovery programs or therapeutic communities is preferred.
