Job Summary

We are seeking a highly skilled Medical Coordination Assistant to join our team at Illumination Foundation. As a Medical Coordination Assistant, you will play a vital role in providing care to our patients and ensuring their smooth transition from hospital to home.

Responsibilities

Our ideal candidate will have a strong background in healthcare and excellent communication skills. The successful candidate will be responsible for:

  • Conducting initial intakes to ensure proper client discharge from hospital with all necessary needs for continued care
  • Assessing medical and mental health status of clients upon arrival, including conducting head-to-toe assessments, checking vitals, and determining insurance status
  • Linking clients to medical services, including insurance, primary care physicians, specialists, and referrals
  • Coordinating home health visits and follow-up medical appointments
  • Promoting health literacy by providing patient education on illnesses, medications, preventative measures, treatment management, and overall general health
  • Exiting clients by providing a comprehensive report of current medications, future medical appointments, insurance information, and other relevant medical details
  • Arranging transportation for clients to and from medical appointments as needed
  • Reporting emergency situations to supervisors and documenting incidents
Requirements

To be successful in this role, you will need:

  • An Associate's degree in a medical field
  • At least 2 years of experience in the healthcare industry
  • Strong verbal and written communication skills
  • Superior customer service skills
  • A high level of professionalism
