We are seeking a highly motivated and compassionate Resident Care Coordinator to join our team at Ovation Heartwood LLC. As a key member of our care team, you will play a vital role in ensuring the well-being and satisfaction of our residents.
Key Responsibilities- Document resident information and communicate changes to relevant parties.
- Maintain accurate and up-to-date resident records.
- Assist with the development and implementation of service plans.
- Coordinate care with outside services and schedule appointments.
- Conduct regular audits to ensure compliance with clinical standards.
- Ensure proper documentation of care plans and resident records.
- Assist with in-service training and orientation programs.
- Support residents with activities of daily living and promote independence.
- Report occupational exposures and participate in Continuous Quality Improvement (CQI) Committee meetings.
- High School Education or equivalent.
- Experience as a CNA and/or medication technician for at least one year.
- Resident Care Coordinator experience or similar preferred.
- Experience with Excel and OnShift scheduling preferred.
- Competitive hourly rate of $22-$26.
