Job Summary

We are seeking a highly motivated and compassionate Resident Care Coordinator to join our team at Ovation Heartwood LLC. As a key member of our care team, you will play a vital role in ensuring the well-being and satisfaction of our residents.

Key Responsibilities
  • Document resident information and communicate changes to relevant parties.
  • Maintain accurate and up-to-date resident records.
  • Assist with the development and implementation of service plans.
  • Coordinate care with outside services and schedule appointments.
  • Conduct regular audits to ensure compliance with clinical standards.
  • Ensure proper documentation of care plans and resident records.
  • Assist with in-service training and orientation programs.
  • Support residents with activities of daily living and promote independence.
  • Report occupational exposures and participate in Continuous Quality Improvement (CQI) Committee meetings.
Requirements
  • High School Education or equivalent.
  • Experience as a CNA and/or medication technician for at least one year.
  • Resident Care Coordinator experience or similar preferred.
  • Experience with Excel and OnShift scheduling preferred.
