Clinical Care Coordinator

Job Summary:
As a Clinical Care Coordinator, you will play a vital role in providing high-quality care to patients in the outpatient setting. Your primary responsibility will be to coordinate and facilitate the clinical management for an assigned population, ensuring that patients receive the best possible care while promoting cost-effective utilization of healthcare resources.
Schedule:
Full-Time, five 8 hour shifts or four 9 hour shifts and one 4 hour shift
Location:
Pacific Street Clinic
Key Responsibilities:
- Facilitate care coordination in collaboration with the patient, family, caregiver, and healthcare team, ensuring that patients receive comprehensive care.
- Assess patient needs and preferences, and work with the medical provider to determine the appropriate level of care.
- Coordinate the interdisciplinary care delivery, ensuring that patients receive timely and effective care.
- Communicate effectively with the healthcare team, family, and patient, ensuring that all parties are informed and engaged in the care process.
- Advocate on behalf of patients and families, ensuring that their needs are met and their rights are protected.
- Evaluate patient outcomes and identify opportunities for improvement, ensuring that care is delivered in a cost-effective and high-quality manner.
Requirements:
- A registered nurse with a current state license and a minimum of one year of experience in an ambulatory healthcare facility or hospital.
- BLS certification and prior working knowledge of using a personal computer and electronic medical records.
