Job Summary:

As a Clinical Care Coordinator at Think Whole Person Healthcare, you will play a vital role in coordinating and implementing care plans for patients under the direct supervision of a healthcare provider. Your primary responsibility will be to work with members of the healthcare team to coordinate patient care services, including preparation of patients for examination or treatment, gathering of information, and performing vital signs, height, weight, and screening procedures.


Key Responsibilities:
  • Coordinate patient care services, including preparation of patients for examination or treatment
  • Perform vital signs, height, weight, and screening procedures
  • Document findings in the electronic health record (EHR)
  • Ensure accurate documentation and relay instructions to patients
  • Collect and/or assist providers in collection of specimens, as required
  • Adhere to infection control standards
  • Assist with special procedures and examinations
  • Maintain organization and cleanliness of examination rooms
  • Complete and monitor required logs for equipment and medication
  • Coordinate patient needs by communicating with various service providers

Requirements:
  • High school diploma or GED required
  • Active LPN license
  • American Heart Associate BLS/CPR certification required

Working Conditions:

This role operates in a healthcare setting. The position requires frequent sitting and computer work, with the ability to vary physical position or activity for comfort.


Physical Demands:
  • Stand 75% or longer during the workday
  • Walk 15% or longer during the workday
  • Sit 10% or longer during the workday
  • Lift and maneuver up to 35 pounds occasionally
  • Bend, squat, kneel, and reach above shoulder level occasionally and twist occasionally
  • Repetitive use of hands for simple grasping, fine manipulation, and computer use
