VNA Health at Home is seeking a compassionate and dedicated Home-Based Palliative Care Clinician to join our team. As a member of our team, you will have the opportunity to make a lasting impact in our community while enjoying the freedom to thrive beyond traditional settings.

Job Summary

The Home-Based Palliative Care Clinician will work closely with the Hospice Team to provide face-to-face assessment services to hospice patients and develop individualized care plans. This role requires strong communication and interpersonal skills, as well as a commitment to providing high-quality patient care.

Salary and Benefits

This position offers a salary range of $130,000 - $160,000 per year, based on experience, as well as a comprehensive benefits package, including medical, dental, and vision coverage, 401(k) matching, and paid time off.

Key Responsibilities:

  1. Provide face-to-face assessment services to hospice patients to assist in determining their continuing eligibility for the hospice program.
  2. Develop individualized care plans in collaboration with the Hospice Team.
  3. Provide education, counseling, and support to patients, families, and caregivers.
  4. Evaluate patient condition and report changes to appropriate personnel and attending physicians.
  5. Participate in quality improvement initiatives and contribute to the development of educational programs for diverse age groups within the hospice program.
Requirements
  • Masters degree in Nursing from an accredited school.
  • Current licensure to practice in the state serviced.
  • Two years of nursing experience, preferably in hospice or palliative care.
  • CPR Certification and compliance with all organizational, local, and state regulatory requirements.
