Job Summary

The Hospice Care Specialist will work closely with the Hospice Team to assess the needs of hospice patients and develop individualized care plans. This role requires strong communication and interpersonal skills, as well as a commitment to providing high-quality patient care.

Salary and Benefits

This position offers a salary range of $125,000 - $155,000 per year, based on experience, as well as a comprehensive benefits package, including medical, dental, and vision coverage, 401(k) matching, and paid time off.

Key Responsibilities:

  1. Conduct thorough assessments of hospice patients to identify their needs and develop individualized care plans.
  2. Collaborate with the Hospice Team to ensure seamless delivery of care and services.
  3. Provide education, counseling, and support to patients, families, and caregivers.
  4. Evaluate patient condition and report changes to appropriate personnel and attending physicians.
  5. Participate in quality improvement initiatives and contribute to the development of educational programs for diverse age groups within the hospice program.
Requirements
  • Bachelor's degree in Nursing or related field.
  • Current licensure to practice in the state serviced.
  • Two years of nursing experience, preferably in hospice or palliative care.
  • CPR Certification and compliance with all organizational, local, and state regulatory requirements.
