We are seeking a compassionate and skilled Registered Nurse (RN) to join our team as a Compassionate Home Health RN. This is an excellent opportunity to make a meaningful difference in the lives of our patients and their families.
About the RoleAs a Compassionate Home Health RN, you will be responsible for providing high-quality patient care in the comfort of their own homes. You will work collaboratively with our multidisciplinary team to develop and implement personalized care plans that meet the unique needs of each patient.
Key Responsibilities:- Conduct comprehensive assessments of patients' physical, emotional, and social needs.
- Develop and implement individualized care plans that address patients' specific needs and goals.
- Collaborate with healthcare professionals to ensure seamless coordination of care.
To be successful in this role, you will need:
- An active RN license in the state of service.
- One year of RN nursing experience or three years of LPN/LVN nursing experience in a home care setting.
- Current CPR certification.
- A valid driver's license.
