As a home health care nurse, you will play a vital role in achieving this mission.
Responsibilities
Your primary responsibility will be to provide high-quality patient care, working collaboratively with our interdisciplinary team to achieve patient outcomes. This includes performing comprehensive assessments, developing and implementing individualized care plans, and educating patients and their families on disease management and prevention strategies.
Requirements
To succeed in this role, you will need an active RN license in the state of service, one year of RN nursing experience, current CPR certification, and a valid driver's license. You should also possess excellent communication and interpersonal skills, as well as the ability to work independently and as part of a team.
