JOB SUMMARY AND QUALIFICATIONS:

The SSVF program is dedicated to assisting Veterans who are experiencing homelessness or are at risk of becoming homeless to resolve their housing crises. The primary objective of the SSVF Case Manager is to support veterans in overcoming their housing challenges, enhance their independent living capabilities through supportive services and education, connect them with community resources, and empower them to achieve long-term housing stability and self-sufficiency.

WHAT YOU SHOULD HAVE FOR THIS ROLE:

This position necessitates a Bachelor's degree in social work or a related field; alternatively, candidates with at least five (5) years of relevant experience without a degree, or Veterans with three (3) years of applicable work experience are also encouraged to apply. Candidates should demonstrate a reasonable combination of the following knowledge, abilities, and skills: family needs assessment, knowledge and ability to access local resources, advocacy, counseling, and collaboration with local service providers. A commitment to the principles of the project is essential. Excellent verbal and written communication skills, a high degree of organization, and creative problem-solving abilities are required. Candidates must possess a personal automobile, a valid driver's license, liability insurance, and be willing to travel between the counties served up to 70% of the time. Completion of required case management and Critical Time Intervention (CTI) training within 90 days of hire is mandatory.

RESPONSIBILITIES:

1. Conduct outreach within the community to raise awareness of SSVF and recruit veteran families into the program.

2. Perform case management duties, including:

a. Conduct assessments and develop/monitor case plans, ensuring necessary follow-up activities are conducted.

b. Establish connections with relevant agencies and service providers in the community.

c. Provide referrals and coordinate with VAMCs and other service providers to deliver healthcare and daily living services to participants.

d. Allocate resources to participants based on assessed needs.

e. Educate participants on available supportive services and their rights.

f. Assist participants in obtaining VA benefits.

g. Aid participants in securing and coordinating other public benefits.

h. Provide or refer participants for supportive services, including personal financial planning, transportation, income support, fiduciary services, legal assistance, childcare, and housing counseling.

i. Complete required documentation (including progress notes) within 48 hours of contact and enter data into the Homeless Management Information System (HMIS).

j. Exercise sound clinical judgment in decision-making regarding participants.

k. Relate to Veterans and their families in a culturally sensitive manner.

l. Fulfill Performance Quality Improvement (PQI) duties as assigned by supervision and the PQI Committee.

m. Facilitate the Critical Time Intervention (CTI) evidence-based model for program participants.

3. Collaborate with other SSVF Case Managers, Intake Coordinators, and Outreach Workers; participate in regular case consultations.

4. Advocate for Veterans, integrating cultural values into their case plans.

5. Act as a liaison between local HUD-VASH teams, attending all case conferences with both HUD-VASH and SSVF teams, and coordinating locally to address the needs of unhoused Veterans in the service area.

6. Understand the challenges faced by clients in both urban and rural settings and develop strategies to effectively serve clients in these environments.

7. Identify systemic barriers and communicate with organizational leadership to collaboratively find viable solutions.

8. Adhere to all policies and procedures of the program and the Council on Accreditation.
