Job Summary

We are seeking a skilled Clinician to join our Adult Community Clinical Services (ACCS) program. As a Primary Clinician, you will work closely with individuals to establish a therapeutic relationship and focus on skill building to support independent housing and community tenure.

Key Responsibilities
  • Develop a collaborative relationship with individuals, family, and other providers to coordinate care and support.
  • Complete person-centered assessments and service planning to ensure effective support.
  • Advocate for individuals and connect them with community resources and services.
Requirements
  • Master's degree in Social Work or Mental Health Counseling required.
  • Licensure required within 2 years of hire (LMHC or LICSW).
  • At least one year of related experience.
  • Excellent written and verbal communication skills, with ability to make effective public presentations.
  • Valid driver's license and available, dependable transportation.
