Job SummaryWe are seeking a skilled Clinician to join our Adult Community Clinical Services (ACCS) program. As a Primary Clinician, you will work closely with individuals to establish a therapeutic relationship and focus on skill building to support independent housing and community tenure.
Key Responsibilities- Develop a collaborative relationship with individuals, family, and other providers to coordinate care and support.
- Complete person-centered assessments and service planning to ensure effective support.
- Advocate for individuals and connect them with community resources and services.
- Master's degree in Social Work or Mental Health Counseling required.
- Licensure required within 2 years of hire (LMHC or LICSW).
- At least one year of related experience.
- Excellent written and verbal communication skills, with ability to make effective public presentations.
- Valid driver's license and available, dependable transportation.
