Position Summary
The Ada County Case Manager plays a crucial role in our Rapid Re-Housing initiative, managing a caseload of families facing homelessness. This position involves collaborating with families to create tailored housing plans aimed at swiftly transitioning them into permanent residences. Following this, the Case Manager will provide guidance and support to help families achieve stability in their lives.

Key Responsibilities

  • Maintain CATCH's impressive success rate of 80% of families achieving long-term housing stability.
  • Foster collaborative relationships with referral sources, community partners, and service providers.
  • Act as a proactive advocate within the Ada County Continuum of Care, promoting Housing First principles and the CATCH model.
  • Report metrics of success and areas for improvement to the Director of Housing Services.
  • Complete a comprehensive 90-day onsite training program.
  • Develop a solid understanding of Rapid Re-Housing models, Trauma-Informed Care, and Motivational Interviewing.
  • Assess program eligibility for applicant families and guide them through the participation process.
  • Collaborate with local landlords to secure safe housing and provide rental assistance.
  • Conduct regular client meetings to evaluate personal and family circumstances, including financial and housing needs.
  • Gather relevant information regarding clients' medical, psychological, and social factors affecting their situations.
  • Provide individual and group counseling to assist clients in mobilizing their resources for improved social functioning.
  • Facilitate referrals to community resources as necessary.
  • Maintain organized client files, ensuring timely updates on employment, medical records, and other pertinent information.
  • Review personal goals with clients and conduct follow-ups to assess service quality and case status.
  • Perform home visits as required.
  • Evaluate clients' eligibility for additional financial assistance from other social service programs.

Qualifications

  • A genuine commitment to ending homelessness, with experience in Housing First programming being advantageous.
  • A Master of Social Work is strongly preferred, though similar qualifications in counseling may be considered.
  • Possession or ability to obtain an Idaho Social Work license.
  • A minimum of three years of experience in case management with vulnerable populations.
  • Excellent written and verbal communication skills, adaptable to various audiences.
  • Proficiency in Google Suite, Microsoft Office Suite, and database management.
  • Reliable transportation and willingness to engage with the community.
  • Preferred knowledge of motivational interviewing and trauma-informed practices.
  • Experience in crisis intervention and client-centered care is a plus.
