We are seeking a highly skilled Advanced Care Clinician to join our team at Optum in San Antonio, TX. As a member of our care delivery team, you will play a vital role in providing high-quality patient care and contributing to our mission of making healthcare better for everyone.
Key Responsibilities- Provide comprehensive, coordinated care to a primarily Medicare-aged population in various clinics, including home, telemedicine, and mobile visits.
- Collaborate with employed, contracted, and affiliated WellMed physicians, advanced practice clinicians, and clinic staff to deliver a comprehensive model of care.
- Educate providers on proper coding, risk adjustment metrics, capturing documentation, and adhering to quality protocols.
- Interview patients to ensure proper documentation and ensure all preventative screenings have been completed.
- Track and report performance metrics related to the strategic direction and operational needs of the organization.

- Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA).
- Current licensure through the Texas Board of Nurse Examiners or licensed physician assistant by the Texas State Medical Board.
- Valid DEA license or ability to obtain one prior to employment.
- 1+ year of clinical experience as a nurse practitioner or physician assistant.
- Comfortable managing a primarily Medicare-aged patient population.
- Familiarity with primary care and chronic condition codes.
- Bilingual (English/Spanish) fluency.
- A working understanding and support of a managed care environment.
