Home and Supportive Care Nurse Practitioner or Physician Assistant

Optum is seeking a Home and Supportive Care Nurse Practitioner or Physician Assistant to join our team in San Antonio, TX. As a member of our team, you will be an integral part of our vision to make healthcare better for everyone.
Key Responsibilities:- Practice with a multi-disciplinary team of healthcare providers, including Primary Care Physicians, RN Case Managers, social workers, and clinic staff.
- Provide comprehensive care to patients in their homes, with the option to provide care in office or other facility settings as needed.
- Manage care for patients with complex medical needs, including those with chronic conditions such as diabetes, hypertension, and kidney disease.
- Diagnose and treat acute and chronic conditions, including minor injuries and infections.
- Facilitate patient and family decision-making regarding treatment decisions, end-of-life care, and serious illness discussions.
- Prescribe pharmacological and non-pharmacological interventions as needed.
- Competitive base salary and annual quality incentive with a target of $20,000.

- Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA).
- Current licensure through the Texas Board of Nurse Examiners or Licensed Physician Assistant by the Texas State Medical Board.
- Valid DEA license or the ability to obtain one prior to employment.
- 1+ year of clinical experience as a Nurse Practitioner or Physician Assistant.
- Comfortable managing a primarily Medicare-aged patient population.
- Reliable transportation.
- 3+ years of clinical experience as a Nurse Practitioner or Physician Assistant.
- Home health experience.
- Bilingual (English/Spanish) fluency.
- A working understanding and support of a managed care environment.
