Job Title: Patient Support Unit Nurse Practitioner or Physician Assistant

Job Summary:

We're seeking a skilled Nurse Practitioner or Physician Assistant to join our Patient Support Unit team in El Paso, TX. As a member of our team, you'll have the opportunity to work with a primarily Medicare-aged patient population, providing comprehensive, coordinated care in a variety of settings, including clinics, home visits, and telemedicine.

Key Responsibilities:
  • Provide high-quality patient care in collaboration with our Contracted and Affiliated WellMed physicians, advance practice clinicians, and clinic staff.
  • Develop and implement care plans that address the unique needs of our patients.
  • Conduct patient interviews to ensure proper documentation and ensure all preventative screenings have been completed.
  • Educate providers on proper coding, risk adjustment metrics, capturing documentation, and adhering to quality protocols.
  • Collaborate with our team to achieve quality and operational goals.
Requirements:
  • Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA).
  • Current licensure through the Texas Board of Nurse Examiners or licensed Physician Assistant by the Texas State Medical Board.
  • Valid DEA license or the ability to obtain one prior to employment.
  • 1+ year of clinical experience as a Nurse Practitioner or Physician Assistant.
  • Comfortable managing a primarily Medicare-aged patient population.
Preferred Qualifications:
  • Familiarity with primary care and chronic condition codes.
  • Bilingual (English/Spanish) fluency.
  • A working understanding and support of a managed care environment.
