Nurse Practitioner or Physician Assistant

Job Summary:We're seeking a skilled Nurse Practitioner or Physician Assistant to join our Patient Support Unit team in El Paso, TX. As a member of our team, you'll have the opportunity to work with a primarily Medicare-aged patient population, providing comprehensive, coordinated care in a variety of settings, including clinics, home visits, and telemedicine.
Key Responsibilities:- Provide high-quality patient care in collaboration with our Contracted and Affiliated WellMed physicians, advance practice clinicians, and clinic staff.
- Develop and implement care plans that address the unique needs of our patients.
- Conduct patient interviews to ensure proper documentation and ensure all preventative screenings have been completed.
- Educate providers on proper coding, risk adjustment metrics, capturing documentation, and adhering to quality protocols.
- Collaborate with our team to achieve quality and operational goals.
- Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA).
- Current licensure through the Texas Board of Nurse Examiners or licensed Physician Assistant by the Texas State Medical Board.
- Valid DEA license or the ability to obtain one prior to employment.
- 1+ year of clinical experience as a Nurse Practitioner or Physician Assistant.
- Comfortable managing a primarily Medicare-aged patient population.
- Familiarity with primary care and chronic condition codes.
- Bilingual (English/Spanish) fluency.
- A working understanding and support of a managed care environment.
