OptumCare is seeking a skilled Home and Supportive Care Nurse Practitioner or Physician Assistant to join our team in San Antonio, TX. As a member of our team, you will have the opportunity to work with a multi-disciplinary team of healthcare providers, including Primary Care Physicians, RN Case Managers, social workers, and clinic staff.
Key Responsibilities:- Provide comprehensive care to patients in their homes, including managing acute and chronic conditions, such as diabetes, hypertension, and kidney disease.
- Diagnose and treat patients with minor injuries and other acute medical conditions.
- Facilitate patient and family decision-making regarding treatment decisions, end-of-life care, and serious illness discussions.
- Prescribe pharmacological and non-pharmacological interventions as needed.
- A competitive base salary plus annual quality incentive with a target of $20,000.
- Optum Care Advanced Provider Clinician Partnership Plan.
- 401(k) with match and UHG Employee Stock Purchase Program.
- 160 hours of PTO, 40 hours CME, and clinics are closed for 8 national holidays.
- Comprehensive benefits from Optum Partner Services.
- Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA).
- Current licensure through the Texas Board of Nurse Examiners or Licensed Physician Assistant by the Texas State Medical Board.
- Valid DEA license or the ability to obtain one prior to employment.
- 1+ year of clinical experience as a Nurse Practitioner or Physician Assistant.
- Comfortable managing a primarily Medicare-aged patient population.
- Reliable transportation.
- 3+ years of clinical experience as a Nurse Practitioner or Physician Assistant.
- Home health experience.
- Bilingual (English/Spanish) fluency.
- A working understanding and support of a managed care environment.
