About the Role

We are seeking a dedicated and compassionate Orthopedic Care Coordinator to join our team at Texas Orthopedic Hospital. This role is responsible for ensuring high-quality, patient-centered care by managing rehabilitative care to achieve optimum outcomes.

The successful candidate will provide program orientation to patients, families, and caregivers, promoting their participation in team discussions related to plans, goals, and status. They will also ensure the implementation of the patient's treatment plan, supporting their strengths, abilities, needs, and preferences throughout the rehabilitation process.

Responsibilities
  • Coordinate patient education and promote participation in team discussions
  • Implement patient treatment plans that support individualized needs and preferences
  • Document findings of Discharge Planning Evaluations (DPE) and psychosocial assessments
  • Communicate social, financial, or discharge needs and preferences of patients and families
Qualifications

To be successful in this role, you will need:

  • A registered nurse diploma, associate degree, or bachelor's degree
  • Basic Cardiac Life Support certification within 30 days of employment start date
  • Registered Nurse licensure within 30 days of employment start date

We offer a competitive salary range of $68,000 - $90,000 per year, depending on experience, as well as a comprehensive benefits package, including:
