About the Job
We are seeking a highly skilled Registered Nurse RN Case Manager to join our team at Texas Orthopedic Hospital. As a key member of our rehabilitation team, you will work closely with patients, families, and caregivers to develop and implement individualized care plans that promote optimal outcomes.
Responsibilities and Qualifications- You will provide expert clinical guidance and support to patients, families, and caregivers, ensuring seamless transitions throughout the rehabilitation process.
- You will collaborate with multidisciplinary teams to coordinate care, communicate effectively, and facilitate timely interventions.
- You will maintain accurate and thorough records of patient progress, identifying areas for improvement and implementing strategies to enhance quality care.
