Primary Care Clinician for Homebound Adults

**Optimizing Quality of Life for Homebound Adults**
At SCAN Health Plan, we're committed to delivering high-quality, patient-centered care to our members. As a part-time in-home visit advanced practice clinician, you'll play a critical role in achieving this mission.
**Responsibilities:**
- Provide primary care services to homebound adults in the comfort of their own homes.
- Drive to patient locations in your assigned geographic area.
- Average 7-10 patient visits per day (if working a full workday).
- Exhibit exceptional communication skills and ability to explain complex medical information in a clear and concise manner.
- Provide health education to patients and work with them to prevent illness, promote health, and manage chronic health problems.
- Refer patients to appropriate medical resources as needed.
- Utilize our electronic medical record (EMR) system to document patient visits accurately and timely.
- Knowledge of Hierarchical Condition Category (HCC) coding.
- Participate in an on-call rotation as required.
**Qualifications and Experience:**
- Graduate of an accredited Physician Assistant or Nurse Practitioner program.
- Current, unrestricted license to practice as a PA or NP in California, including DEA license.
- Minimum of 1 year of experience in primary care, hospice, or urgent care/outpatient setting.
- Previous experience in a home health, hospice, or other home visit setting is highly desirable.
- Minimum of 1 year of previous experience utilizing an EMR system with a good working knowledge of HCC coding.
- Strong computer skills, including MS Office and Outlook.
- Excellent verbal and written communication skills.
- Current driver's license, auto insurance, and reliable transportation.
- Spanish speakers are strongly preferred.
