We offer a range of services, including preventative care, chronic condition management, and laboratory tests, all delivered directly in residents' homes. Our primary care team works closely with staff to address individual health concerns and provide rapid access to medical care through same-day or next-day appointments.
The OpportunityWe are seeking a part-time or full-time Primary Care Provider to join our team in the East Bay area. This role will involve delivering care to a smaller patient panel of complex care patients, contributing to a value-based care model, and working with a motivated team to provide high-quality patient care.
This person-centered care model leverages the power of our care navigators to maximize the time and energy of our providers. As a Primary Care Provider, you will be responsible for developing and executing detailed care plans, working with our Medical Director to identify and develop policies, and collaborating with our clinical team to provide high-quality care.
Key Responsibilities- Provide onsite primary care services to older adults living in Senior Living Communities
- Develop and execute detailed care plans on all primary care patients
- Work with Medical Director to identify and develop policies to address issues that arise
- Collaborate with Primary Care Coordinators to provide monthly Chronic Care Management visits and care
- Work with Traveling Clinical Assistants to support diagnostic needs for each patient
- Run regular huddles and provide leadership to the clinical team
- Identify ways of working with senior living management and care teams to better carry out Pine Park care plans and improve patient outcomes
- Board-Eligible or Board-Certified Physicians in Internal Medicine or Family Medicine OR Nurse Practitioner or Physician Assistant - preferably with geriatrics experience
- Comfortable with a fast-paced and changing environment
- Willingness to provide onsite clinical care across the East Bay
- Ability to travel to partner communities as needed
- Desire to work with complex care patients and support the building of a holistic care model
- Strong orientation towards collaboration and working closely with a multidisciplinary team
- Strong written and verbal communicator
- Ability to work with tech-enabled care delivery tools
